If you are a new patient requesting an appointment, please fill out the following form. A team member will reach out to you to schedule within 48 business hours.

Please have a credit card available when making new patient appointments. There is a fee of $100 for appointments that are not cancelled 24 hours prior to scheduled appointment.

Please arrive 15 minutes prior to your appointment time & bring the following with you:

• Current insurance card(s)
• Photo ID
• Referral (HMO plans only)
